Pneumectomy enlarged to carina es seldom performed having high morbi-mortality therefore it may be indicated in selected patients.A multidisciplinary team including pathologists, anaesthesists must work coordinatively.En la cirugía por cáncer de pulmón la resección ampliada a la vía canalicular se plantea en dos situaciones: lobectomía ensanchada a bronquio fuente y neumonectomía ensanchada a carina. Se establecen las indicaciones de la lobectomía con resección anastómosis brónquica. Se define el candidato ideal y se concluye que existen indicaciones de necesidad y de elección. En la mayoría de los casos se realiza en los lóbulos superiores, fundamentalmente en el derecho.Las neumonectomías ensanchadas a carina son operaciones que se realizan con baja frecuencia y tienen una elevada morbimortalidad. Deben indicarse en pacientes rigurosamente seleccionados. Se insiste en la estrecha cooperación de un equipo integrado con anestesistas y patólogos.
