| Summary: | Analysis of an uncommon therapeutic approach in arterial occlusive diseases of lower limbs: surgery limited to the profunda femoris artery. Eleven procedures were performed on 10 patients: 9 endarterectomies with vein patches and 2 pure angioplastiae with vein patches. In 6 cases endarterectomy and complementary vein patch were performed in the initial segment of the superficial femoral artery, in order to preserve its patency. Results were satisfactory in 9 cases, with clear improvement of the ischemia symptoms. The anatomical, surgical, pathological and physiological basis of this procedure are discussed. The indications of this technique derive from the impossibi!ity of using the conventional methods to restare circulation (by pass grafts or direct disobstruction of the arterial axis). From the different technical variants described, endarterectomy and closing with a vein patch is preferred. |
