Vascular injuries need an aggressive approach to repair both vessels. lf only there is carried out the arterial repair, with ligation of the corresponding vein, the defered venous repair must be made, specially when extense venous thrombosis is present. On hand of 28 vascular injuries by gunshot of extremities, authors systematize therapetic measures. |

La cirugía reconstructiva por heridas arteriales ha sido llevada a cabo con éxito en los últimos 15 años. No sucedió lo mismo con las heridas venosas, quegeneralmente han sido ligadas, más que reparadas. La ligadura favorece la trambosis venosa profunda extensa. Es ésta más que la ligadura simple, la que condiciona una importante alteración en la circulación de retorno, favoreciendo la trombosis de una arteria, que había sido reparada concomitantemente, o de unaarteria que sufrió lesiones intimales mínimas, que primitivamente no hubieran determinado su obstrucción. Ante la presencia de heridas de los paquetes vascularesprincipales, la reparación de ambos troncos debe ser la meta. Si simplemente se lleva a cabo la reparación arterial, complementada por ligadura dela vena homóloga, debe realizarse la plastia venosa, mismo diferidamente, con más razón si se agrega una trombosis venosa profunda.
