En la piel del pezón y aréola, la presencia de las grandes célulasclaras de Paget, intraepidérmicas. Dicha célula es neoplásica y maligna. Existe en los conductos terminales del pezón una proliferación epitelial atípica intraductal,que también es carcinomatosa. Finalmente, si el tiempo es suficiente, la presencia de un carcinoma profundo mamario, de tipo común. Para un grupo de autores la célula dePaget procede de un cáncer de los conductos del pezón y ha invadido la epidermis de la zona. Para otro grupo, dicha célula es autóctona de la epidermis y enconsecuencia coexiste con la lesión de los conductos y eventualmente con el cáncer mamario profundo.
