Summary:Lipomas are the most common benign soft tissue tumors. Osteolipoma is an extremely rare histological variant of lipoma that contains mature lamellar bone within the tumor and is even more rare if it has no direct connection to bone structures. We present the case of a 45-year-old patient who came to the clinic for noticing a painless inguinal mass. Preoperative imaging tests found a space-occupying lesion that required surgical excision. The Pathology report revealed a giant osteolipoma independent of bone structures, an entity with very few cases published in the bibliography
